LPN/LVN - Nurse Navigator
About the role
The ACO LVN Navigator is a licensed clinical LVN who functions as a liaison between the patient’s primary care provider, the patient or caregiver, and the multidisciplinary healthcare team to assist in developing an optimal plan of care. This role involves assessing needs and coordinating resources to promote optimal health benefits and outcomes.
Responsibilities
- Assess needs and coordinate resources to support optimal health benefits and outcomes.
- Provide clinical and social assessments in multiple care settings, including over the phone, emergency department, primary care provider’s office, skilled nursing facilities, and in rare cases, the patient’s home.
- Build rapport with primary care providers, office staff, patients, and their families to support and expedite timely transitions of care and appropriate utilization.
- Ensure effective communication of a patient’s treatment plan and general status to all caregivers during all transitions of care.
Requirements
- Graduation from an accredited LPN/LVN certification program.
- Licensed by the State of Texas as an LVN.
- Minimum of two years’ experience as an LPN/LVN.
- Effective communication in English, both verbally and in writing.
- Ability to document in different electronic health records as well as utilize Microsoft Products.
Qualifications
- High school diploma or equivalent.
- Current Texas LVN license.
- Experience in a healthcare setting preferred.
Skills
- Strong interpersonal and communication skills.
- Ability to work independently and as part of a team.
- Knowledge of healthcare policies and procedures.
- Proficiency in electronic health records and Microsoft Office products.

Pay
The salary range for this position is $50,000 - $60,000 annually, commensurate with experience.
