- 40 hours weekly
- Monday - Friday, day shift
- Option to work 8 or 10-hour shifts

Responsibilities
Act as a liaison between patients, professional staff, and physicians by providing care management and coordination for adult and pediatric patients with complex conditions, social needs, and educational needs. Serves in an expanded healthcare role to collaborate with primary care providers, specialists, and patient/families to ensure the delivery of quality and cost-effective healthcare services. Assesses, plans, implements, coordinates, monitors, and evaluates all options and services with the goal of optimizing the patient's health status and decreasing hospital and emergency room utilization. Support quality improvement initiatives and adherence to safety standards.
Requirements
- Licensed RN in the State of Michigan
- Experience in complex chronic disease management preferred
- Bachelor of Science degree in nursing preferred
- Five years' experience as an RN preferred
