About the Role
The Obstetrics Nurse Navigator provides individualized clinical navigation, patient education, and care coordination for obstetric patients across the ambulatory care continuum. This role serves as a central point of contact for patients, families, providers, nursing, scheduling, authorization, and ancillary teams to support timely access to care, follow-up, and clear communication.
The Nurse Navigator identifies and helps address barriers to care, coordinates referrals, diagnostic testing, procedures, and follow-up needs, and supports patients through pregnancy, postpartum, and other obstetric care pathways. The role contributes to quality, patient experience, care gap closure, and operational efficiency by promoting proactive outreach, organized follow-up, and coordinated interdisciplinary care.
