Nurse Navigator - Cardiovascular Admin - FT
Position Summary
Provides care coordination for transitions across the continuum of inpatient, outpatient, surgical and invasive procedural settings. Assists with collection of data necessary to support the patient safety and performance improvement plan and progress toward established performance measures. Works as a clinician to support high quality, timely care delivery. Acts a resource across the Medical Center relative to cardiac and vascular surgical patient care.

Responsibilities
- Collaborates with interdisciplinary teams to ensure seamless patient care transitions
- Coordinates care across inpatient, outpatient, surgical, and invasive procedural settings
- Supports patient safety initiatives and performance improvement goals
- Works as a clinician to provide expert care to cardiac and vascular surgical patients
- Identifies and searches for information to support patient care and performance improvement
- Synthesizes information to form recommendations for patient care and process improvements
Requirements
- Minimum 2 years’ experience in quality, patient safety, infection control, or performance improvement methodologies
- Experience working with Cardiovascular surgical patients
- Experience coordinating care for patients with complicated medical diagnoses
- Excellent written and verbal communication skills in English
- Current California RN license
- Current BLS certification
- Computer familiarity using Word, Outlook and Excel, or similar database software
Qualifications
- Bachelor of Science in Nursing
- Ability to maintain a work pace appropriate to the workload
- Ability to establish and maintain effective working relationships across the Health System
- Ability to design and implement patient and family education programs
Skills
- Strong written, oral communication, and problem-solving skills
- Resourcefulness to identify and search for information
- Ability to synthesize information to form recommendations
