Nurse Navigator

About the role

The RN Navigator at Valley Children's Maternal Fetal Center serves as a clinical expert, patient advocate, and care coordinator for patients receiving services through the Maternal Fetal Center. They work collaboratively with physicians, advanced practice providers, nurses, and the interdisciplinary healthcare team to coordinate and oversee patient care throughout the pregnancy journey.

Responsibilities

• Identify patient needs and coordinate care across multiple specialties and healthcare settings
• Facilitate communication among providers, patients, and families
• Collaborate across departments and with community providers to ensure seamless transitions of care
• Support high-quality clinical outcomes
• Advocate for patients and families while navigating complex healthcare needs

Requirements

• Associate Degree in Nursing required
• Bachelor's Degree in Nursing (BSN) preferred
• Current California Registered Nurse (RN) license required
• Minimum of two (2) years of experience in a related clinical nursing specialty preferred
• Experience in maternal-fetal medicine, obstetrics, women's health, or care coordination is highly desirable

Compensation: $60.15–$85.81 per hour
