Responsibilities
- Completes a comprehensive health assessment of the patient’s physical, psychological, social, environmental, financial and functional status in collaboration with multidisciplinary team members.
- Includes assessment of past and present treatment course.
- Aids in organizing and coordinating timely patient appointments.
- Understands and develops goals, treatment plan and provider options in concordance with evidence-based standards and clinical trials.
- Works directly with physician lead to review diagnostic studies, clinical history and course of treatment to identify most appropriate next course of care.
- Collaborates with multidisciplinary team members in performance improvement activities including assessment, planning, implementation and evaluation of clinical outcomes and processes for patients.
- Uses critical thinking and problem-solving skills to analyze and synthesize data and formulate an appropriate case management plan to address the patient’s individual and family needs.
Requirements
- Bachelor’s degree in Nursing
- Current State of Michigan Registered Nursing License
- CPR or BCLS Certified
- Three years of experience in an acute care setting
- One year of experience in medical, surgical or radiation oncology (for oncology specialty)
