Nurse Manager II, Inpatient Care Coordination
About the role
This position is responsible for leading the implementation of best practice models, patient care delivery systems and practice standards across the service line. This includes delegating tasks related to the investigation and resolution of patient/family/member concerns regarding patient care and services, facilitating collaboration with physicians, outside healthcare providers, and other health care team members to achieve optimal and safe patient care across the continuum, and proactively monitoring resource needs in several clinical areas to ensure appropriate assignment and utilization.

- Lead the development and implementation of plans, policies, and processes for data gathering, using relevant data gathered, and analysis
- Lead and implement long-term strategic projects designed to remediate issues and improve quality, service, affordability, and/or operating efficiency

Qualifications
- Bachelor's degree in Nursing (BSN)
- Current Registered Nurse (RN) license in good standing
- Minimum 5 years of progressive nursing experience, preferably in an inpatient care coordination role
- Experience with evidence-based practice and patient care delivery systems
- Experience with patient care coordination and care at home operations
- Experience with managing resources and monitoring compliance
- Experience with leading multidisciplinary teams and facilitating project teams
- Experience with managing and implementing long-term strategic projects
- Experience with leading and implementing data gathering and analysis
- Experience with leading and implementing root cause analysis and solution implementation
