Purpose
The primary purpose of your job position is to coordinate and supervise nursing services provided to the participants in the PACE Organization (PO). Services may be delivered in the day center or in participants' home.

Pay
Pay Range: $40.49-50.61/hr
Responsibilities
- Supervision of center and/or in-home staff, including orientation, competency assessment, training, development, and performance appraisal.
- Care coordination with outside contracted service providers, including hospitals, nursing facilities, assisted living facilities, labs, oxygen, etc.
- Facilitation of new participant integration into the health care delivery system, including medication, immunizations, routine monitoring of chronic problems, and nursing care plan development.
- Participation in survey preparation and correction plans for department deficiencies.
- Adherence to all regulations governing PACE organizations and licensed adult day care centers.
- Reviewing participant medical records for timely and accurate staff documentation.
- Managing infection surveillance, prevention, and control in collaboration with the health system Infection Control Officer.
- Communicating, counseling, and guiding participants and their families regarding service needs and concerns; making appropriate referrals.
Requirements
- Current, unencumbered active license to practice as a Registered Nurse in the state of PACE Organization.
- Bachelor’s degree in nursing (BSN) required.
- Active CPR certification required.
- Minimum of three (3) years' health care experience with emphasis in geriatrics.
- Minimum of one (1) year documented experience working with a frail or elderly population or appropriate training if less than one (1) year of experience.
- Previous supervisory experience required.
- Valid driver's license and proof of insurance.
Qualifications

- Minimum of one (1) year documented experience working with a frail or elderly population or if less than one (1) year of experience, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.

- Valid driver's license and proof of insurance.
