Monday - Friday 8:00a - 5:00p (some weekends and holidays)
Annual Salary
$54,318 + $3,000 Sign-On Bonus
Position Summary
The Nurse LPN - ACT Program supports our Assertive Community Treatment team by delivering safe, timely nursing services for adults with severe and persistent mental illness. This role is essential to maintaining medication continuity, clinical stability, and accurate documentation in a community-focused model of care.
Key responsibilities
- Psychiatric nursing assessments
- Medication administration (including narcotic dispensing as applicable)
- Therapeutic support
- Crisis response assistance
- Maintaining complete medical records in accordance with regulatory and program standards
Required Qualifications/skills
- Current Licensed Practical Nurse (LPN) license with the Virginia State Board of Nursing
- Graduate of an accredited LPN school of nursing with one (1) year of medication administration experience in substance abuse treatment, psychiatric, or long-term care settings, including dispensing of narcotic medication
- Valid driver’s license for community-based client support
- Strong communication skills and ability to work effectively on a multidisciplinary team

Your Typical Day
- Review updates, prioritize client needs, and align the plan with team members
- Balance scheduled client interactions with walk-in needs, adjusting calmly when symptoms escalate or a situation becomes urgent
- Spend focused time ensuring records are current and complete, then connect with colleagues to confirm next steps and continuity of care
- Clear handoffs, documentation checks, and preparation for the next day’s caseload at the end of the day
