Responsibilities
- Identifies and manages appropriate hospital and facility discharges.
- Provides backup for Intake Coordinator.
- Performs community education, liaison, and outreach activities.
- Develops and manages relationships with health partners and organizations.
- Takes hospice referrals and coordinates with discharge planners.
- Conducts General Inpatient Assessments, Evaluations, and admissions as needed.
- Provides general information to patients and families, anticipating and eliminating potential conflicts.
- Sets up and attends Team Meetings, AHS Ethics Committee, and Newton Palliative Care Meetings.
- Submits reports on activities and accomplishments to the Administrator.
- Maintains data for the agency’s contacts and outreach efforts.
- Participates in planning and preparation of new endeavors, such as Palliative Care.
- Establishes long and short-term goals for Hospice.
- Completes assignments in a timely, thorough, and detailed manner.
- Seeks assistance when workload exceeds a reasonable level and seeks opportunities to refine expertise.
- Keeps abreast of technical and legal developments.
- Maintains relations with Atlantic Health Services and St. Luke's through involvement in Palliative Care Services.
- Serves as a resource to organizations on Hospice and Pain Management.
- Adheres to employment policies, maintains confidentiality, and adheres to HIPAA regulations.
- Complies with the dress code policy and maintains a professional appearance.
- Performs other duties as assigned.
Qualifications
- Availability for 4 shifts per month, including weekend availability.
Pay
$47.00 per hour.
