Nurse Coordinator - Hematologic Malignancy

This Nurse Coordinator role will work with our hematologic malignancy program supporting one lymphoma provider. Hours: Monday - Friday 8 am - 5 pm, potential work from home option one day per week after a year. Full time, salaried & benefits eligible.

Role Purpose
Captures the health care of select patient populations/designated service across the continuum of care. Coordinates the delivery of safe and appropriate patient care governed by evidence-based clinical practice. Monitors care and data to optimize patient outcomes. Facilitate the education of patients and families. Acts as a resource to the health care team. Ensures actions meet and support the overall goals of the business.
Responsibilities
- Facilitates patient care by assessing and evaluating patient's physical, psychosocial and emotional needs.
- Captures the delivery of safe and appropriate patient care governed by evidence-based clinical practice.
- Makes sure care and data are monitored to optimize patient outcomes.
- Facilitates the education of patients and families.
- Acts as a resource to the health care team.
- Ensures actions meet and support the overall goals of the business.
Minimum Requirements
- Education: Nursing Diploma/Associate's Nursing
- Experience: 2-5 years
- Licenses & Certifications: RN Preferred
Preferred Qualifications
- Education: Bachelor's Degree Nursing
