Nurse Case Manager - Sacramento, CA

About the role

The Field Nurse Case Manager role covers the Sacramento County Area in CA, and surrounding areas. This position involves home-based work with travel, up to 2 hours one-way (4 hours round trip per appointment). Responsibilities include assessing, planning, coordinating, implementing, and evaluating the medical case of work-related injuries. The Case Manager collaborates with insurance carriers, medical care providers, attorneys, employers, and employees, monitoring progress and reporting results.

• Assessing and managing the medical needs of work-related injuries
• Coordinating with insurance carriers, medical providers, attorneys, employers, and employees
• Maintaining timelines and adhering to organizational protocols
• Implementing and evaluating treatment plans
• Reporting progress and results to employers and insurance carriers

Requirements

• Current, unencumbered RN license in California
• Experience in case management of workers' compensation cases, preferred
• Recommended certifications (CCM, CRRN, CDMS, CRC) within two years of hire date

Qualifications

• PC and technology skills
• Communication skills (written, verbal, and interpersonal)
• Ability to operate autonomously with minimal oversight
• Skilled at patient education
• Valid driver’s license, and good driving history
