Nurse Case Manager - South Bay Area
About the role
We are seeking a Field Nurse Case Manager to cover the South Bay area in San Francisco, CA, and surrounding areas. This is a home-based position with travel up to 2 hours one-way (4 hours round trip per appointment). Appointments may include visits to doctors’ offices, hospitals, and other locations, with multiple appointments possible in one day.
The Case Manager is responsible for the medical case management of work-related injuries, including assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals. You will work with insurance carriers, medical care providers, attorneys, employers, and employees, closely monitoring the progress of the injured worker and reporting results back to the employer and insurance carriers.

Requirements
- Current, unencumbered Registered Nurse (RN) license in California.
- Experience in case management of workers' compensation cases (preferred).
- Recommended certifications (CCM, CRRN, CDMS, CRC) strongly preferred; must obtain within two years of hire date.
- Strong organizational and timeline adherence skills.
- Proficiency with PC and technology.
- Excellent written, verbal, and interpersonal communication skills.
- Ability to operate autonomously with minimal oversight.
- Skilled at patient education.
- Valid driver’s license and good driving history.
