Position Summary
Primary Responsibilities
- Collaborates with the physician and Interdisciplinary Team to assess, plan, implement, and evaluate the plan of care for patients and their caregivers
- Utilizes principles of palliative care and holistic philosophy to provide holistic care
- Provides direct supervision of Hospice Aides during care delivery
- Offers case management services to support patient care needs, ensuring alignment with care goals and treatment plans
- Provides case management services to best benefit patients and caregivers
- Care provided reflects compliance with federal and state regulations, along with Hospice Hawai’i’s policies and procedures
- Documents all nursing and case management activities in accordance with hospice policies, procedures, and regulatory standards
- Keeps the attending physician informed of changes in patient condition
- Functions as primary nurse within the framework of an Interdisciplinary Team
- Requests support services from other Interdisciplinary Team members as appropriate
- Demonstrates good judgment in seeking and accepting guidance; facilitates clear and honest communication with other team members
- Attends and actively participates in Interdisciplinary Team meetings and conferences
- Maintains assigned productivity standards and manages a patient caseload as directed
- Participates in assigned on-call rotations, including evenings, weekends, or holidays
- Provides end-of-day report to the RN on-call at the end of each workday
- Travels as required for patient/family care
- Supports and participates in Quality Improvement, Performance Improvement, and Compliance initiatives in accordance with direction from the clinical leadership team
- Attends required in-service education and professional development activities
Knowledge And Skills Requirements
- Ability to exercise sound, independent judgment in planning and providing care
- Flexible and organized in planning
- Skilled in providing holistic nursing care to patients and their families
- Able to identify and provide for the psychosocial needs of caregivers in concert with the other members of the Interdisciplinary Team
- Able to set priorities, research, problem-solve, and make independent decisions with minimal supervision by using logic to identify key facts, explore alternatives, and propose quality solutions within the scope of the job
- Able to multi-task with the ability to meet multiple deadlines and possess strong organizational, planning, and follow-through skills
- Excellent written and verbal communication skills as well as the ability to communicate clearly, concisely and effectively with all levels of individuals
Experience And Education Requirements
- Valid Registered Nurse License in the State of Hawai’i
- Bachelor of Science in Nursing (BSN) preferred
- 1 year of experience as a staff nurse in an acute hospital
- Hospice and/or home health nursing experience preferred
Employment Requirements
- A legally compliant background check
- Drug screen & physical exam
- Valid Hawai’i Driver’s License
- TB clearance
- CPR & BLS Certification
Working Conditions
- Generally, indoors in-patient homes or Hospice Hawai’i homes, nursing, and acute care facilities; may include outdoors in all types of weather
- Work Location: This position is based in the field, requiring travel to patient homes, facilities, and offices throughout Oahu using a personal motor vehicle
- Hours: Generally, Monday through Friday 8am-4:30pm
