CASE MANAGER (RN) - FULL TIME
Responsibilities
Texoma Medical Center, a 414-bed acute care facility, collaborates with patients, patient family members, and healthcare professionals to design, implement, and evaluate plans of care specific to each patient's needs. This position works under the direction of the Director, Case Management.
Requirements
- Education - Per RN Licensure Requirements. Bachelor's Degree Preferred.
- Experience - At least three (3) years previous clinical and/or case management/utilization management experience, preferably in a high volume acute care hospital/healthcare facility.
- Licenses - Valid, current RN state license or compact.
- Skills - Excellent interpersonal, communication, and problem-solving skills necessary to gather and exchange data, both internally and externally, with key medical professionals, patients, and patient family members. Ability to effectively utilize critical thinking and problem-resolution skills. Must be able to effectively communicate with key community stakeholders to appropriately assist patients.
