About the role

Paradigm is an accountable specialty care management organization focused on improving the lives of people with complex injuries and diagnoses. We are hiring a Field Nurse Case Manager to support injured workers throughout Charlotte and surrounding communities. This is a hybrid position requiring regular local travel (80% of the time) for in-person patient appointments. Candidates must be located in the Raleigh area and licensed in NC. The Case Manager is responsible for the medical case management of work-related injuries, including assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals. The role involves working with insurance carriers, medical care providers, attorneys, employers, and employees, and closely monitoring the progress of injured workers with reporting to employers and insurance carriers. Candidates must be willing to learn and take on catastrophic cases.

Responsibilities

  • Conduct medical case management for work-related injuries
  • Perform assessments, planning, coordination, implementation, and evaluation of injured/disabled individuals
  • Collaborate with insurance carriers, medical care providers, attorneys, employers, and employees
  • Monitor progress of injured workers and report results to stakeholders
  • Travel locally for in-person patient appointments (80% of the time)
  • Handle catastrophic cases

Qualifications

  • Current, unencumbered compact Registered Nurse (RN) license in NC
  • Registered as a Rehabilitation Professional with the North Carolina Industrial Commission required within 60 days of hire
  • Experience in case management of workers comp cases, preferred
  • Recommended certifications (CCM, CRRN, CDMS, CRC) preferred; required within two years of the date of hire

  • Strong communication skills (written, verbal, and interpersonal)
  • Ability to operate autonomously with minimal oversight
  • Skilled at patient education
  • Valid Driver’s License with good driving history
