Nurse Case Manager

Role and Responsibilities

Provide medical case management, screening, education, counseling, medication management, and community healthcare networking for youth in residential programs.

Participate in a rotating on-call schedule primarily offering phone consultations to ensure accessibility without on-site presence.

Qualifications and Skills

• Must hold a Bachelor or Associate of Science in Nursing, be licensed as a Registered Nurse in Illinois, and possess a valid Illinois Driver's License.
• Experience with psychiatric care and children/adolescents is preferred.
• Strong organizational, communication, and clinical skills are essential.

Key Duties
