Nurse Case Manager
About the role
We are seeking a Field Nurse Case Manager to cover Queens, Brooklyn, Bronx and surrounding areas to include Mt. Vernon, Manhattan, Staten Island, Yonkers, and North Bergen (NJ). This is a home-based position with travel, up to 2 hours one-way (4 hours round trip per appointment). Appointments may be at doctors’ offices, hospitals, and various other locations, with multiple appointments possible in one day.
This individual is responsible for the medical case management of work-related injuries, including assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals. The Case Manager works with insurance carriers, medical care providers, attorneys, employers, and employees, closely monitoring the progress of the injured worker and reporting results back to the employer and insurance carriers.

Qualifications
- Current, unencumbered Registered Nurse (RN) license in NY
- Experience in case management of workers comp cases, preferred
- Recommended certifications (CCM, CRRN, CDMS, CRC) preferred; REQUIRED within two years of the date of hire

- PC and technology skills
- Communication skills (written, verbal, and interpersonal)
- Ability to operate autonomously with minimal oversight
- Skilled at patient education
- Valid Driver’s License and good driving history
