About the role
Nurse Case Manager supporting youth with emotional, psychological, and physical needs in a residential setting.
Responsibilities
- Provide medical case management, screening, education, and counseling services.
- Manage medications and coordinate with community healthcare providers.
- Participate in a rotating on-call schedule, primarily via phone consultations.
Qualifications
- Bachelor's or Associate of Science in Nursing from an accredited program.
- Current Illinois RN licensure and valid driver's license.
- Preferred experience in psychiatric care and working with children/adolescents.
- Strong organizational, communication, and clinical skills.
