Nurse Case Manager

About the role

We are seeking a Nurse Case Manager to provide comprehensive healthcare services within a residential youth support setting.

Responsibilities

• Medical case management, screening, education, and counseling
• Medication management and coordination with community healthcare providers
• Participating in a rotating on-call schedule, primarily via phone consultations

Qualifications

• Bachelor or Associate Degree in Nursing from an accredited program
• Current Illinois RN licensure and valid driver’s license
• Experience with psychiatric care and children/adolescents preferred
• Strong organizational, communication, and clinical skills
