Nurse Case Manager

About the Role

Provide medical case management, screening, education, counseling, medication management, and community healthcare networking to youth in residential programs. Participate in a rotating on-call schedule primarily offering phone consultations to ensure accessibility.

Responsibilities

• Support youth with emotional, psychological, and physical needs
• Coordinate with healthcare providers
• Engage in educational and counseling activities

Requirements

• Bachelor or Associate Degree in Nursing from an accredited program
• Current Illinois RN licensure and driver's license
• Preferred experience with psychiatric care and youth
• Strong organizational, communication, and clinical skills
• Must pass a physical and background check
