Nurse Care Manager (Residential)

About the role

Located in Western Montgomery County, PA, this role offers a flexible Monday-Friday schedule. We are seeking a compassionate Nurse Care Manager to join our healthcare team.

Responsibilities

• Coordinate, implement, and manage healthcare services for an assigned caseload of individuals with intellectual and developmental disabilities.
• Ensure continuity of care through medical appointment coordination, health monitoring, staff education, and interdisciplinary collaboration.
• Collaborate with residential teams, providers, families, and community partners to improve health outcomes and quality of life.
• Conduct biweekly community home visits, meetings, hospital visits, medical appointments, and other field-based responsibilities as needed.

Requirements

• Valid RN/LPN License in the state of PA.
• Valid Driver's License.
• Experience working with individuals with intellectual disabilities.
• Current Healthcare Provider CPR.

Qualifications

• Clear criminal record and other background checks in accordance with state requirements.
• Annual certification in Healthcare Provider CPR.

Skills

• Strong communication and interpersonal skills.
• Ability to work independently and collaboratively.
• Knowledge of intellectual and developmental disabilities.
• Proficiency in healthcare coordination and management.
