NP/PA, Patient Connect - Full Time

About the Role

This autonomous role offers the opportunity to impact patient care positively while working across diverse care settings, with a focus on improving transitions, reducing readmissions, and supporting patients and families during critical health phases.

Responsibilities

• Perform thorough assessments and accurately document findings per CMS regulations
• Ensure correct ICD10 coding and complete documentation of diagnoses
• Review medications and address quality measures, including advanced care planning
• Communicate with patients, families, and healthcare team members regarding medical concerns
• Complete mandatory training and travel between care sites as required
• Contribute to program enhancements through independent thinking and initiative

Requirements

• Active, unrestricted licensure and certification (e.g., ANCC, AANP, NCCPA)
• Reliable transportation and ability to navigate varied environments
• Proficient in electronic medical records and documentation
• Ability to work independently with minimal supervision

Preferred Qualifications

• Clinical experience in geriatric, long-term care, or home care settings
• Familiarity with complex behavioral and social needs
