Base salary: $100,000.00-$125,000.00
CME Allowance: $3,000

Requirements
- BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required (American Heart Association or American Red Cross accepted)
- Nurse Practitioner credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date required
- Drug Enforcement Admin credentialed from the Drug Enforcement Administration preferred
- Registered Nurse preferred
- Masters Degree of Nursing required
- Board Certified Nurse Practitioner or Physician Assistant
- Eligible for state medical license and DEA/Controlled Substance registration
