About the role
Join a patient-centered, home-based care team as an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant).
Responsibilities
- Conduct comprehensive in-home health assessments, including history reviews, physical exams, screenings, and point-of-care testing.
- Identify and document diagnoses, communicate findings to primary care providers, recognize urgent situations, educate members on health topics, and address social determinants of health.
Requirements
- Hold active, unrestricted licensure, relevant certifications, and a valid driver's license with reliable transportation.
