About the role

The African American Planning Commission (AAPCI) is a nonprofit organization dedicated to reducing homelessness and addressing related issues such as domestic violence, HIV/AIDS, mental illness, substance abuse, affordable housing shortages, and unemployment.

Responsibilities

• Participate on the intake screening assessment team to determine suitability for independent living.
• Provide individualized mental health counseling and lead resident population group sessions.
• Use a Person-Centered approach to help tenants set goals, identify interests, and address concerns.
• Get to know tenants and their psychosocial history to provide strategic interventions and services.
• Offer education and modify service plans based on tenant progress.
• Attend and participate in educational activities and department meetings.
• Direct tenants to appropriate community agencies and resources for financial assistance, counseling, rehabilitation, auxiliary care, or medical supplies.
• Prepare, write, and maintain required progress notes, reports, and group notes.
• Engage all members of the family to improve family functioning.
• Conduct visits in tenants' units to ensure compliance with lease and program rules.
• Maintain contacts with various community agencies to gather information and improve services.
• Help tenants understand and address diagnoses (medical, psychiatric, substance abuse).
• Advocate and connect tenants to medical, mental health, and other supportive counseling services.
• Engage with community partners to provide services onsite and offsite.
• Conduct unit inspections to ensure compliance with lease and program rules.
• Attend all internal trainings, development conferences, and other mandated trainings.
• Guide tenants through the application, qualification, and service processes for SSI, SSDI, Medicaid, Medicare, HRA, HPD, SNAP, and other services.
• Manage an assigned caseload of tenants, ensuring comprehensive case management services are provided.

Qualifications

• Minimum Qualifications: Bachelor’s degree in a related field (social work, mental health counseling, psychology, sociology) with at least 2-3 years’ experience in a residential setting.
• Experience with one or more populations including mental health, substance use, inpatient/outpatient treatment, homelessness, and supportive housing.
• Strong verbal and interpersonal communication skills with a focus on tenant services.
• Demonstrated ability to interact effectively and collaboratively with a diverse community of residents, program staff, and external partners.
• Ability to exercise good judgment and apply problem-solving skills.
• Experience working collaboratively in a team-oriented and outcomes-focused environment.
• Ability to monitor and manage a variety of tasks assigned.

Compensation and Benefits

The base salary compensation range for this role is $60,000.00 per year. AAPCI offers a competitive benefit package which includes:
