Responsibilities

• Provide skilled nursing care to patients in their homes
• Develop individualized care plans and perform comprehensive assessments
• Administer medications and treatments
• Educate patients and caregivers on care management
• Coordinate care with physicians and the interdisciplinary team
• Play a critical role in helping patients recover safely, manage chronic conditions, and avoid unnecessary hospitalizations
• Build meaningful relationships with patients and families while promoting independence and improving outcomes
• Ensure compassionate, patient-centered care

Requirements

• Current California Registered Nurse (RN) license
• Current BLS certification
• Valid driver’s license, reliable transportation, and auto insurance
• Excellent assessment, communication, and critical thinking skills
• Ability to work independently while collaborating with an interdisciplinary team
• OASIS documentation experience

Preferred Qualifications

• Previous experience working in home health, hospice, or a similar caregiving environment
• Familiarity with KanTime Electronic Medical Record (EMR) system
• Bilingual skills
