Responsibilities
- Work closely with physicians, caregivers, and healthcare professionals to deliver individualized care plans that meet each client's unique needs
- Assess patient conditions
- Administer medications and treatments
- Monitor health status
- Educate patients and families
- Communicate important clinical updates to the care team
- Apply clinical judgment to help patients remain safe and comfortable at home
Requirements
- Active Texas nursing license (LVN)
- Current in-person CPR/BLS certification
- Valid Texas driver's license
- Current automobile insurance
- Recent TB test or chest X-ray
- At least one year of nursing experience in a home health agency, healthcare facility, primary care clinic, or similar setting
- Strong commitment to providing high-quality, patient-centered care
