Non-Medical Case Manager - Truman
About the role
UNM Medical Group, Inc. is hiring for a Non-Medical Case Manager to join our Truman Health Services team. This opportunity is a full-time day shift opening located in Albuquerque, New Mexico. Minimum $41,305 - Midpoint $51,621* Salary is determined based on years of total relevant experience. Salary is based on 1.0 FTE (full time equivalent) or 40 hours per week. Less than 40 hours/week will be prorated and adjusted to the appropriate FTE.
Responsibilities
- Coordinates patient documentation to maintain eligibility for a variety of core medical and social support services;
- Conducts patient interviews required to complete intake applications and all required forms for core medical and social support services;
- Works closely with medical case managers to reduce patient barriers to obtaining needed medical care and /or social services;
- Afford assistance in the identification of community resources that build support for medically and socially complex patients;
- Maintain current information on all regularly accessed community resources;
- Perform a range of administrative functions required to ensure efficient program operation;
- Maintain professional growth and development through completion of required departmental and New Mexico Department of Health competencies;
- Perform miscellaneous job-related duties as assigned.
Requirements
High School diploma or GED and 2 years of related experience or a certification as a Certified Community Health Worker (CHW). Completed degree(s) from an accredited institution that are above the minimum requirements may be substituted on a year for year basis. Verification of education and licensure will be required if selected for hire.
