Non-Medical Case Manager

Key Responsibilities

To provide HIV non-medical case management to make sure Ryan White Part B clients enter and remain in HIV medical care and have access to and are adherent on their HIV medications.

Provide case management in accordance with the Case Management Service Standards for the Utah Ryan White Part B Program

  • Perform client intake
  • Aid in application and re-certification for the Part B Program
  • Assess needs utilizing assessments as provided by the Program
  • Develop care plan
  • Cookordinate care and benefits
  • Provide and track referrals
  • Provide internal and external resources
  • Perform follow-up as needed and indicated by acuity and assessment scores.

Qualifications

  • Minimum or "day-one" qualifications: One year of paid employment/professional experience providing case management services, excluding internships, AND one of the following:
    • Bachelor of Social Work.
    • Bachelor degree in health or human services.
    • OR Five years of paid employment/professional experience providing case management services, excluding internships.
  • Working knowledge of web-based applications and Google Workspace, Microsoft Office, or similar.
  • Ability to work independently with minimal supervision

Preferred Or Trainable/on-the-job Qualifications

  • Previous case management experience
  • Working knowledge of HIV
  • Knowledge of or experience with Ryan White Part B
  • Familiarity with social service and support systems within Utah
  • Experience with ClientTrack data management solution
