About the role
Under the general supervision of the Charge Nurse and in accordance with facility policies and procedures, this position provides direct care to residents, assists with activities of daily living, and supports the highest level of resident function. The role involves maintaining resident comfort, cleanliness, and safety while reporting needs and concerns to the supervisor on duty.
Responsibilities
- Performs or assists with resident direct care needs, including lifting, turning, positioning, transporting, bathing, dressing, oral hygiene, grooming, and responding to call lights.
- Provides restorative care such as range of motion exercises, ambulation, and bowel and bladder training.
- Maintains cleanliness of resident rooms, including daily and deep cleaning, and discharge cleaning.
- Engages residents in activities of interest and implements restorative nursing programs to maintain ADLs.
- Communicates professionally and effectively with residents, peers, families, managers, and visitors.
- Reports changes in residents' physical or mental conditions in a timely manner.
- Assists nurses with tasks such as measuring and recording vitals, gathering documentation, making beds, cleaning resident areas, and assisting with new admissions.
- Helps residents exercise their rights and reports complaints or concerns to the Charge Nurse or supervisor.
- Documents resident information accurately and confidentially in the electronic medical record system.
- Adheres to safety, compliance, infection control, and equipment use protocols.
- Performs food service functions, including preparing residents for meals, serving food, assisting with eating, and recording food/fluid intake.

- Reports complaints, allegations of abuse, or misappropriation of funds/resident property to appropriate supervisors.
- Completes annual education requirements, including compliance, privacy, security, and HIPAA training.
Requirements
- High school diploma or GED preferred, or currently enrolled in a program to achieve preferred education.
- Two to four years of experience preferred.
- Good communication, comprehension, and interpersonal skills.
- Ability to speak, read, write, and understand English.
- Working knowledge of Windows, Microsoft Office Suite, and the World Wide Web.
Physical demands
- Lifting/Carrying: Frequent 10–25 lbs., Sometimes 26–50 lbs., Frequent 51–75 lbs., Rare 76+ lbs.
- Stooping, climbing, pushing, pulling, standing, walking, and driving are frequent.
- Sitting is rare.
- Exposure to disagreeable conditions such as noise, odors, illness, infection, and physical contact with disruptive residents.
