Responsibilities
- Serve as the main point of contact for residents and families regarding care needs.
- Manage resident charts and care updates, ensuring accuracy and clear communication with the team.
- Lead care conferences with residents and families to ensure care plans align with their needs and preferences.
- Manage caregiver and med tech schedules, finding coverage for open shifts.
- Be in the rotation for the Staffing Phone
- Be on-call med-tech shifts.
- Assist in conducting continuing education and training for employees.
- Aid in counseling and coaching employees as needed.
Qualifications
- At least 18 years of age
- Experience as a Medication Technician in a long-term care setting
- Experience creating comprehensive care plans tailored to individual needs (preferred)
- Ability to effectively present information and respond to questions from groups or managers, residents, employees, and the general public
- Ability to interpret and communicate a variety of instructions furnished in written, oral, electronic, or schedule form
- Ability to read, analyze, and interpret charting standards, physician orders, or administrative regulations
- Ability to write reports, business correspondence, and procedure manuals
- Proficient PC skills including e-mail, record keeping, routine database activity, word processing, spreadsheet, graphics, etc.
