Night Shift Licensed Practical Nurse (LPN)

About the role

We are seeking a compassionate Licensed Practical Nurse (LPN) to provide direct resident care in a skilled nursing facility. You will assist residents with activities of daily living, ensuring their comfort, dignity, and safety, while fostering a culture centered on learning, spirituality, and intra-preneurship.

Responsibilities

• Administer medications and perform wound care and skilled nursing procedures.
• Supervise and guide nursing assistants and medication aides.
• Monitor residents' physical and emotional states; notify physicians of any changes.
• Develop, implement, and review personalized care plans in collaboration with the interdisciplinary team.
• Ensure accurate clinical documentation.
• Coordinate resident admissions, transfers, and discharges.
• Participate in quality assurance and staff education activities.

Requirements

• Active LPN/LVN license in the practice state.
• CPR/BLS certification preferred.
• At least one year of clinical nursing experience; long-term care experience preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of long-term care regulations.
• Effective communication and clinical judgment skills.
• Experience with electronic health records and Microsoft Office.
