LPN - Licensed Practical Nurse (NIGHT SHIFT)

Night Shift, 16 hours per week, every other weekend

About the Role

Provides nursing care under the supervision of an RN for assigned patients in assigned areas, practicing Evidence-Based nursing as the standard.

Responsibilities

• Provides nursing care appropriate to the age, needs, and abilities of the resident/patient, including:
  • Medication and treatment administration
  • Teaching
  • Implementation of the interdisciplinary care plan
• Contributes to the care plan development by collecting and recording data in an ongoing and systematic manner
• Communicates and collaborates effectively with residents/patients, families, colleagues, and other members of the interdisciplinary team to meet resident/patient and unit needs
• Documents accurate, pertinent, and comprehensive information in a timely and concise manner as required per regulation and facility policies and procedures
• Competently uses Electronic Medical Record
• Supervises the care provided by the CNAs to assure compliance with the interdisciplinary care plan and resident profile
• Participates in educational programs, in-services, meetings, and workshops, and utilizes current literature to enhance professional skills and knowledge
• Attends all annual mandatory in-services
• Participates in quality improvement programs as directed
• Follows standards of the Diligent Minimal Lift Program
• Performs other duties as required

Requirements

• Graduate of an accredited school of nursing for practical nursing
• 1+ years clinical experience in a hospital or similar health care setting preferred
• Current State of CT license to practice as a licensed practical nurse

• Proficient in speaking, reading, and writing English to communicate with patients, residents, and clinical staff
• Effectively interact with residents and staff
• Organized and detail-oriented
• Ability to utilize a personal computer/CRT to input and retrieve patient information
• Familiarity with care plan development
• Provides care and services demonstrating behaviors and principles of person-centered care, including:
  • Affording people dignity, respect, and compassion
  • Offering coordinated care, support, and treatment
  • Providing care that is personalized
