Responsibilities

• Perform patient assessments, medication administration, wound care, and chronic disease management
• Respond promptly to urgent situations
• Collaborate with on-site and on-call healthcare providers
• Maintain accurate electronic medical records (EMR) documentation
• Deliver high-quality patient care and support healthcare team collaboration to promote patient well-being

Requirements

• Valid LPN license
• Compassionate, detail-oriented, and able to work effectively in a team environment
• Strong communication and clinical skills
