Responsibilities

• Complete the questionnaire thoroughly and provide all requested information.
• Indicate N/A for questions that are not applicable and provide an explanation if necessary.
• Upload supplemental information sheets if additional details are needed.
• Upload certificates verifying the firm's status as per the requirements.
• Provide a list of any judgments, arbitrations, liens, insurance claims, or lawsuits within the past 5 years.
• Answer questions regarding criminal background information, including convictions and charges.
• Provide insurance coverage details and state willingness to purchase additional insurance if required.
• Describe the firm's Health & Safety Program, including written programs, site-specific plans, inspections, investigations, and training.
• Detail the firm's safety practices, such as drug and alcohol screening, first aid, and safety meetings.
• Respond to questions about fall protection, electrical safety, confined space training, and other relevant safety measures.
• Report on the firm's compliance with OSHA regulations and the number of employees trained in OSHA 10-Hour and 30-Hour programs.
