Salary: $52,000 with uncapped monthly bonus structure; top performers earn 20%+ of base salary (approx. $10.5K+ annually).

About the role

The New Member Engagement Coordinator builds relationships and engages new members by locating, supporting with social services, educating on AbsoluteCare services, and enrolling them into programs. This role establishes trust and meaningful relationships with AbsoluteCare members and communities, meeting members where they are—including in the home, community, hospital, or office setting—through telephonic and in-person outreach.

Responsibilities

  • Conduct community outreach telephonically and in person to locate, engage, and educate members on AbsoluteCare’s services, including enrolling in PCP or ancillary services such as Care Management or Behavioral Health services.
  • Conduct research and in-person outreach to locate difficult-to-contact enrollees to increase assessment completion and participation in clinical programs.
  • Conduct in-person non-clinical assessments to understand enrollee care needs, preferences, and socioeconomic barriers, and evaluate the home environment as needed.
  • Support members to improve their wellbeing by helping them stay out of the hospital and attend regular visits to their primary physician.
  • Assist members in engaging with mental health and substance use treatment as needed.
  • Support members to ensure pick-up of their prescriptions.
  • Provide member education on community resources and benefits.
  • Partner with the care team (community, providers, internal staff) to build trust and rapport with members and their support systems.
  • Effectively communicate AbsoluteCare’s services, benefits, and how they can help the specific member being engaged.
  • Connect members with appropriate AbsoluteCare resources if they do not wish to change their PCP to AbsoluteCare.
  • Develop new and innovative ways to build trust and engagement with members who are located but not engaged, as well as locate members not yet found.
  • Identify barriers to care that may negatively impact engagement in care or healthcare goals and support addressing those needs, including connecting members to community resources.
  • Promote AbsoluteCare’s 5-star customer service philosophy in the medical center and community.
  • Report and enter daily outreach activities into CRM and electronic health and/or medical records.

Requirements

  • High school diploma or equivalent required.
  • Valid driver’s license, proof of insurance, and reliable personal vehicle.
  • 2+ years of experience working with a community agency providing services in health care, including community resources, communication skills, individual and family advocacy, health education, and service coordination.
  • Experience and passion for working with and engaging vulnerable, hard-to-engage, and complex populations.
  • Independent and persistent self-starter.
  • Proficient in PC skills, such as Microsoft Office.
  • Ability to travel locally at minimum 80% of the time.
  • Knowledge and trusting relationships within the local community, and experience navigating community resources.

Qualifications

  • Certified as a Community Health Worker (CHW) or eligible and planning to obtain CHW certification preferred.
  • Active CPR certification.

Working Conditions

This job operates in the community, in members’ homes, and in the AbsoluteCare clinical center. The role routinely uses general office equipment and requires the use of a personal vehicle to conduct outreach, including member home visits.

Physical Requirements

  • Ability to communicate clearly and exchange accurate information constantly.
  • Ability to remain stationary for long periods of time.
  • Constantly operates computer, keyboard, copy and fax machines, phone, and other general office equipment.
  • Ability to occasionally move objects up to 20 lbs.
  • Driving and walking in the community where members live.
