About the Role

The Concussion Nurse Navigator is a key member of the Concussion Program Team, serving as a liaison for the community.

Responsibilities

• Assess patients and families using established guidelines to identify needs.
• Develop and implement community education plans tailored to patient needs.
• Coordinate care by facilitating referrals to supportive services and community agencies, monitoring responses.
• Support program development, analyze data accuracy, and provide feedback for team performance.
• Enhance patient experience by ensuring standards align with service excellence policies.
• Advocate for professional development and effective team participation.
• Assist with medical record management and data abstraction as needed.

Requirements

• Bachelor’s in Nursing required; Master’s preferred.
• Minimum of 3 years in nursing, with experience in neurosciences, outpatient care, community nursing, or related fields.
• Strong assessment, communication, and organizational skills.
• Ability to negotiate and adapt to community needs.
• Physical demands include regional travel to support community locations.
