Nephrology Physician

About the role

We are seeking an experienced Medical Director with a background in Cardiology, Nephrology, or Primary Care to lead an integrated clinical care model focused on primary care coordination and specialty collaboration.

Responsibilities

• Provide clinical and administrative leadership to a multidisciplinary team including nurse practitioners, physician assistants, and nurse case managers.
• Drive population health outcomes by reducing hospitalizations, emergency visits, and readmissions.
• Support primary care integration, clinical quality, and develop a strong referral network.
• Oversee provider workflows, implement evidence-based protocols, and facilitate team feedback and case reviews.
• Promote equitable care through outreach to underserved communities and strategic growth planning.
• Serve as a clinical liaison with partners, payers, and health system leaders.

Requirements

• MD or DO with board certification in relevant specialties.
• Active or eligible Georgia medical license.
• At least 2 years of clinical leadership or administrative experience.
• Knowledge of managed care, quality metrics, and population health principles.
• Strong communication, collaboration, and problem-solving skills.
