Responsibilities
- Ensures optimal drug therapy for patients in the Neonatal ICU.
- Provides educational activities to pharmacy staff and other caregivers.
- Develops and implements patient-specific treatment plans with neonatologists, nurses, and multidisciplinary care teams.
- Participates in system-wide clinical meetings and initiatives to support medication management and standardization.
- Serves as a clinical resource for the healthcare system on neonatal pharmacotherapy, including dosing, safety, and evidence-based practice guidance.
- Develops, reviews, and implements system-wide neonatal medication protocols, policies, and order sets.
- Supports antimicrobial stewardship, nutrition support, and other specialized neonatal care programs.
- Provides education and training to pharmacy staff, residents, and other healthcare professionals on neonatal pharmacotherapy topics.
- Maintains and evaluates patient outcomes to ensure safe, effective, and high-quality pharmaceutical care.
Requirements
- Completion of an ASHP approved pharmacy residency program.
- Three (3) years practicing pharmacist experience.
- Licensed as a registered Pharmacist in Connecticut or eligibility to receive licensure within 45 days of hire date.
Qualifications
- Bachelor's degree in Pharmacy.
- Pharm.D. preferred.
