Job Summary
Responsible for developing and coordinating initiatives directed at the appropriate use of antimicrobials within the organization. This includes developing, implementing and maintaining antimicrobial stewardship program, daily patient care monitoring and continuous performance improvement. Responsible for interdisciplinary education regarding antimicrobials and AMP programs.
Responsibilities
- Develops, implements and maintains antimicrobial stewardship program
- Daily patient care monitoring and continuous performance improvement
- Interdisciplinary education regarding antimicrobials and AMP programs
- Double checks and continues to review neonatal and pediatric doses of medications and IV fluids to ensure all weight-based doses are appropriate
- Reviews NICU patient’s vaccination status and helps keep patients up-to-date on required vaccinations
- Responds to all pediatric codes during pharmacy open hours
- Attends Pharmacy and Therapeutics Committee meetings and Department of Pediatrics Committee meetings
- Attends NICU interdisciplinary Unit Practice Council Meetings
- Manages pharmacy staff development, competency and training in NICU/PICU/PEDS programs in collaboration with the Pharmacy Clinical Manager
- Provides patient/parent education of newly diagnosed type 1 diabetic patients
- Provides medication discharge instructions for NICU parents when NICU patients are getting close to being discharged
- Verifies correct labeling on outpatient medications for NICU parents
- Provides pediatric medication safety initiatives/information to nursing/medical staff via verbal communication or pharmacy newsletter
- Provides education and training to pharmacy students
- Performs pharmacist duties as assigned
Qualifications
- Doctor of Pharmacy or Master’s degree required
- Pharmacy residency training or Board Certification with added qualifications in pediatrics preferred
- Dedicated Pediatric Lead pharmacist with the equivalent of > 5 years’ experience providing pharmaceutical care for neonates and/or pediatric patients
- PGY1 Residency with emphasis on pediatrics within a pediatric hospital or PGY2 Residency with Pediatric specialty
- Board Certified Pediatrics
- Maintain a knowledge of NALS in regards to appropriate medication, dose, and administration
- Registered pharmacist licensed in the State of Florida
