The Neighborhood Physician provides primary care for high-acuity Medicare Advantage patients within a dedicated panel, functioning as the primary care provider of record. This role partners with an interdisciplinary team to manage complex, high-utilization patients through a combination of clinic-based and home-based care. Care is delivered in a referral-based model, focused on reducing hospitalizations, improving outcomes, and supporting longitudinal care management. The physician collaborates with advanced practice clinicians, case management, and social work to address medical and psychosocial needs, while supporting quality initiatives including HEDIS, RAF accuracy, and patient experience.
