NEIGHBORHOOD HEALTH - NURSE PRACTITIONER (APRN) OR PHYSICIAN ASSISTANT
About the role
Hartford HealthCare’s Neighborhood Health program is seeking passionate per diem Advanced Practitioners to join our primary care team dedicated to improving health equity in underserved communities. This role offers the opportunity to provide high-quality preventative and urgent care through mobile and community-based services, addressing health disparities and promoting wellness.
Shift Detail
This position requires travel to our home base hubs out of Hartford and New Britain then travel in company fleet to clinic sites and Street Medicine. Schedule: Full-time, weekdays only. Call schedule: No call required.

Setting
100% Outpatient Primary Care/Urgent Care – Family Medicine
Qualifications
- Licensure: Unrestricted State of Connecticut Medical License (or eligible)
- Training: Family Nurse Practitioner (FNP) or Physician Assistant (PA)
- Experience: 2+ years as a Family Nurse Practitioner or Physician Assistant, preferably in a primary care or urgent care setting.
- Valid driver’s License within 3 Months (90 days) of hire date or job transfer date
- Ability to push/pull/lift 50 lbs
Requirements
