About the role

The Structural Heart Nurse Navigator functions on the multidisciplinary team as a clinical expert, patient advocate, educator, coordinator, and liaison for all structural heart patients and their respective family members. Responsible for ensuring all patients navigating within the structural heart program receive quality and comprehensive services. Coordinates patient care throughout the continuum in collaboration with the multidisciplinary team. Serves as a liaison throughout the continuum of care to the patient/family as well as point contact for team members to include physicians, mid-level providers, RNs, and other department staff members. Serves as the single point of contact for referring physicians, patients, and other caregivers to provide resources and assistance with accessing clinical and supportive care services offered within the Structural Heart Program.

Responsibilities

  • Ensures all patients navigating within the structural heart program receive quality and comprehensive services.
  • Coordinates patient care throughout the continuum in collaboration with the multidisciplinary team.

Requirements

  • Montana Registered Nurse License
  • National Provider BLS – American Heart Association

Qualifications

  • Bachelor’s degree in Nursing
  • Cardiovascular specialty certification
  • 2 years of cardiac clinical nursing experience

Skills

  • Excellent communication skills
  • Strong organizational and time management skills
  • Ability to work collaboratively with a multidisciplinary team
  • Knowledge of cardiovascular procedures and treatments

$40.37 - $62.66 per hour
