Navigator II
Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of programs throughout Arizona, including residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services. About the role
The Navigator II’s primary role is to enhance the likelihood of success, identify and reduce barriers for clients, and connect them with needed services and resources. The Navigator II will work with individuals who need additional support and engagement, conducting outreach, case management, and ongoing wrap-around supportive services. Responsibilities include mapping out appropriate next steps for individuals who may need assistance accessing various services to achieve ongoing success. The Navigator II is part of a multi-disciplinary team of internal and external stakeholders and will document and track required patient interactions according to regulatory and accreditation standards.
Responsibilities
- Conduct outreach and engagement with clients requiring additional support.
- Provide case management and ongoing wrap-around supportive services.
- Identify and reduce barriers to client success.
- Connect clients with needed services and resources.
- Map out appropriate next steps for individuals accessing services.
- Document and track patient interactions in compliance with Arizona Administrative Code (AAC), ADHS/DBHS Provider Manual, CARF, and CBI policies.
Requirements
- High school diploma or GED (required).
- Associates Degree (or higher) in a field related to Behavioral Health (preferred but not required).
- Must be in recovery from Alcohol or Drugs and/or GMH for equal to or greater than 1 consecutive year.
- Minimum of ninety (90) days experience in a Navigator I or Peer Support Specialist role (required).
- 1-3 years of full-time healthcare-related work experience (preferred but not required).
- 1-3 years of substance abuse/mental health recovery-oriented service (preferred but not required).
- Current AZ Driver’s License (valid and in good standing).
- Clear 39-month Motor Vehicle Record.
- Behavioral Health Technician (BHT) certification in accordance with A.A.C. R9-10-101.33 and CBI Clinical Policies and Procedures.
- Arizona Fingerprint Clearance card (program-specific).
