About the role

Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services throughout Arizona. The Navigator II's primary role is to enhance the likelihood of success, identify and reduce barriers for clients, and connect them with needed services and resources. This position works with individuals determined to need additional support and engagement, conducting outreach, engagement, case management, and ongoing wrap-around supportive services. The Navigator II maps out appropriate next steps for individuals needing assistance accessing various services, and is part of a multi-disciplinary team of internal and external stakeholders.

Responsibilities

  • Conduct outreach and engagement with clients
  • Provide case management and ongoing wrap-around supportive services
  • Map out appropriate next steps for individuals needing assistance accessing services
  • Document and track required patient interactions according to Arizona Administrative Code (AAC), ADHS/DBHS Provider Manual, CARF standards, and CBI Policies and Procedures
  • Collaborate as part of a multi-disciplinary team

Requirements

  • High school diploma or GED
  • Must be in recovery from Alcohol or Drugs and/or GMH for equal to or greater than 1 consecutive year
  • Minimum of ninety (90) days experience in a Navigator I or Peer Support Specialist role
  • Current AZ Driver's License (valid and in good standing)
  • Clear 39-month Motor Vehicle Record
  • Behavioral Health Technician (BHT) in accordance with A.A.C. R9-10-101.33 and CBI Clinical Policies and Procedures
  • Arizona Fingerprint Clearance card (program specific)

Qualifications

  • Associates Degree (or higher) in a field related to Behavioral Health (preferred, not required)
  • 1-3 years of full-time Health Care related work experience providing adequate exposure to behavioral and medical crisis situations (preferred, not required)
  • 1-3 years of substance abuse/mental health recovery-oriented service (preferred, not required)
