Navigator II

About the role

The Navigator II’s primary role is to enhance the likelihood of success, identify and reduce barriers for clients, and connect them with needed services and resources. The Navigator II will work with individuals who have been determined as in need of additional support and engagement, conducting outreach, engagement, case management, and ongoing wrap-around supportive services. The Navigator II will map out the most appropriate next steps for individuals who may need assistance accessing various services to achieve ongoing success. This role is part of a multi-disciplinary team made up of internal and external stakeholders.

The Navigator II will document and track required patient interactions according to the Arizona Administrative Code (AAC), Arizona Department of Health Services/Department of Behavioral Health Services (ADHS/DBHS) Provider Manual, Commission on Accreditation of Rehabilitation Facilities (CARF), Community Bridges Policies and Procedures, and program-specific requirements.

Responsibilities

• Conduct outreach and engagement with clients in need of additional support.
• Provide case management and ongoing wrap-around supportive services.
• Identify and reduce barriers to accessing services and resources.
• Map out appropriate next steps for individuals to achieve success.
• Document and track patient interactions according to regulatory and organizational standards.

Requirements

• High school diploma or GED (required).
• Associate degree (or higher) in a field related to Behavioral Health (preferred but not required).
• Minimum of six months of recovery from substance use and/or mental health disorders (required).
• 1-3 years of full-time healthcare-related work experience, with exposure to behavioral and medical crisis situations (preferred but not required).
• 1-3 years of substance abuse/mental health recovery-oriented service (preferred but not required).
• Behavioral Health Technician (BHT) certification in accordance with A.A.C. R9-10-101.33 and CBI Clinical Policies and Procedures (required).
• Minimum of ninety (90) days of experience in a Navigator or Peer Support Specialist role (required).
• Current Arizona Driver’s License (valid and in good standing).
• Clear 39-month Motor Vehicle Record.
• Arizona Fingerprint Clearance Card (program-specific).
