MSW - Home Health
Position Summary
This position provides psychosocial support and resource coordination for patients receiving home health services. The Social Worker partners with patients, families, caregivers, nurses, and interdisciplinary team members to assess needs, connect individuals with community resources, and help address barriers that may impact health, safety, and quality of life. The role is primarily field-based, requiring travel to patients' homes throughout the service area.
Responsibilities
- Conduct psychosocial assessments in patients' homes
- Evaluate social, emotional, financial, and environmental factors affecting patient care
- Develop and implement individualized care plans
- Provide counseling, support, and crisis intervention as appropriate
- Assist patients and families with adjustment to illness, disability, or life transitions
- Connect patients to community resources, including:
- Financial assistance programs
- Transportation services
- Housing resources
- Caregiver support
- Community-based services
- Collaborate with physicians, nurses, therapists, and other healthcare professionals
- Advocate for patient needs and coordinate community services
- Document assessments, care plans, interventions, and outcomes according to regulatory standards
- Educate patients and families regarding available resources and support systems
- Manage a caseload while prioritizing visits and meeting documentation requirements
Qualifications
- Master's Degree in Social Work (MSW) required in Oregon
- Excellent time management skills
- Compassion and empathy
- Ability to establish professional boundaries
- Strong knowledge of community resources (or ability to quickly learn them)
- Comfortable working independently in the field
- Strong organization and documentation skills
Schedule
Monday through Friday, 8:00 AM - 4:30 PM. No weekends listed. 13-week assignment. Start date ASAP.
