MSSP Supervisor (2)

About the role

The MSSP Supervisor provides direct supervision of Social Work Care Managers (SWCM) and Nurse Care Managers (NCM). This position reports to the MSSP Site Director and works 40 hours per week in Monrovia, CA.

Responsibilities

• Provides direct supervision of SWCM & NCM.
• Provides ongoing consultations and recommendations regarding participants and services.
• Conducts annual performance evaluations.
• Troubleshaots any issues with service provision with vendor, participants, caregivers and family.
• Reviews and approves all participant care plans.
• Reviews progress notes and conducts chart audits.
• Conducts weekly team conference meetings with staff.
• Oversights and authorizes purchases utilizing Waiver Services including verification that other resources have been exhausted.
• Monitoring and tracking of waiver expenditures.
• Ensures implementation of policies and procedures.
• Ensures implementation of guidelines provided in the Site Manual.
• Screens applicants and assigns referrals as needed to ensure meeting monthly census goals.
• Submits billing to Medi-Cal as needed to support the team & program.

Qualifications

• Master’s Degree in social work, psychology, nursing, counseling, gerontology, sociology or rehabilitation.
• Two (2) years of direct experience working with the elderly.

Pay

The expected pay range for this position is $38.00 - $42.00 per hour. The actual compensation will be determined based on experience and other factors permitted by law.
