About the role

This full-time position offers the opportunity to provide patient-centered care for hospitalized patients with serious illness while partnering with physicians and an interdisciplinary team to improve quality of life and support patients and families through complex medical decision-making.

Responsibilities

• Manage symptom control
• Engage in goals-of-care conversations
• Develop and implement care plans
• Collaborate with interdisciplinary team members
• Participate in ongoing education and professional development

Requirements

• Board Certified Nurse Practitioner (Acute Care NP preferred; Family NP considered based on experience)
• AZ license (or ability to obtain)
• Experience in Palliative Care, Hospice, Hospital Medicine, Internal Medicine, Oncology, Geriatrics, or a related specialty preferred
